The Offerings of a Herbal Physician

The primary role of the Herbal Physician is to help educate and support you as you work towards your goals to optimal health and wellness. The Herbal Physician neither diagnoses nor treat dis-eases. Instead, they make assessments of imbalances based on energetic patterns and physiological representation, and educate you on how the ways to support the healing of your mind, spirit and physical structure.

By signing your new client forms, you acknowledge and agree:

I fully understand that Earth Shanti is not a "Licensed Medical Doctor", does not diagnose or treat dis-eases, and that I am not here for medical, diagnostic or treatment procedures.
The services performed by Earth Shanti, whether in person, email, phone or virtual communication, are at all times restricted to consultation on the subject of wellness. These services are solely intended to provide me with resources to promote my own health and well-being. Her services do not involve diagnosing, treatment or prescription of remedies for the treatment of dis-ease.
I fully understand that it is my individual right to decide how I choose to care for my health. Earth Shanti, has not suggested that I cease any current medical care that I am receiving to include: drug therapy, x-ray treatments, chemotherapy, surgery or any other medical procedures that my medical doctor or any other health practitioner deems necessary for my health. If I choose to follow the recommendations made by medical doctor or other practitioners, I understand that such decision is my responsibility and will not hold any other persons responsible for any consequence of such a decision.
I am here of my own accord at his visit, and any other subsequent visits, and not as an agent for federal, state or local government agencies on a mission of entrapment or investigation.
I understand that all information discussed will be confidential.
I understand that any therapies I undertake at The Whole Initiative, LLC are undertaken of my own free will. I accept that the ultimate responsibility for my health is my own and that The Whole Initiative, LLC is here to support me in this.
I understand that my Herbal Physician reserves the right to determine which cases fall outside their scope of profession, in which event an appropriate referral will be recommended. I hereby agree to assume full responsibility for any manner of loss, injury, claim or damage whatsoever, known or unknown, incurred as a result of same and I, my heirs and executors, administrators or assigns for any loss, injury, claim or damage sustained as a result of my attendance and/or participation. I have read the above release and waiver liability, and fully understand its contents and voluntarily agree to the terms and conditions stated.

***IMPORTANT***

Chronic illnesses and chronic symptoms do not show up overnight and neither does healing. There are no overnight fixes, guaranteed results. Your results will be a direct reflection to the level of your commitment to bettering your well-being. No refunds are given on products once purchased so please take this into consideration when making any purchases.

There will be support via email in-between appointments. If you have any questions about your wellness plan or need clarity on anything discussed in your appointment, please reach out. Please keep longer questions for our next appointment together. If any new symptoms, or you have questions not covered in your appointment, you will have to book another appointment.

I respond to most emails with 24 business hours, however it can take up to 48 hours in certain instances as I do not respond to emails on the Sabbat (Fridays 6pm to Saturday 6pm).

I will not respond to health or wellness plan related questions on any social media platforms, as they are not a secure platform; please contact me via email with any wellness related questions.
